Position Overview:

As a Care Manager, you will be coordinating care for patients who are returning home from nursing homes or rehabilitation facilities. This job entails extensive communication with patients over the phone, therefore, being personable, compassionate, and understanding are important traits. We want to assist our patients with enhancing their health & overall well-being by building lasting connections with them. This position reports to the Care Manager Supervisor.

General Responsibilities (including, but not limited to):

Researching & coordinating various services such as physical therapy, medical supplies, transportation, etc., in alignment with patients needs.

Extensively communicate with health providers, patients, key contacts, to ensure continuity of services.

Document patient notes in the electronic health record in an accurate, timely, and detailed manner for approximately 50 patients per month.

Follow-up on phone calls, emails, and correspondence promptly to ensure quality standards of service delivery are met.

Perform other duties as assigned.

High Call Volume- Must be comfortable being on the phone for the majority of the day calling patients, vendors and setting up appointments for patients.

This position is work from home, and you will be expected to speak to the patients over the phone, you will not have face to face interactions with patients.

Setting up Mental Health appointments with patients, candidate must be comfortable speaking about various chronic conditions such as depression, anxiety, and anything else the patients may be struggling with.
