As a Care Manager, you will:
- Work with families to identify present and future care needs.
- Create and implement a personalized care plan.
- Conduct home visit, routine and as needed.
- Coordinate care ongoing, including coordinating medical and paramedical care, hiring/supervising caregivers, applying for benefits, etc.
- Liaison with family and helps facilitate decision-making.
- Manage crisis situations.
- Advocate for your clients!

Requirements
- 2+ years experience working with seniors in Israel.
- Proficiency in both English and Hebrew.
- Excellent communication & listening skills.
- Comfort with using cloud-based applications and programs.

- Hourly compensation, flexibility in job scope (at least 50% part-time up to full time)
